  • Contact Information: Name, phone number, email address, and mailing address.
  • Health Information: Any Protected Health Information (PHI) shared as part of care coordination and transition management services.
  • Communication Data: Messages and information provided through mobile text messages or other digital communication channels.

Important Note: No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent will not be shared with any third parties.

In relation to any instances that Kava Health uses SMS communications. The following apply.

  • Service Description. By providing your mobile phone number, you as a client consent to receiving SMS messages from Kava Health related to its care coordination and transition management support.
  • Message Frequency. Message frequency may vary.
  • Message and Data Rates. Standard message and data rates may apply depending on your carrier.
  • Opting Out. Clients may opt out of receiving SMS messages from Kava Health by replying “STOP” to any SMS message they receive from us. After opting out, a confirmation message will be sent out, and Kava Health will no longer send SMS messages to your number.
  • Help and Support. Clients that need assistance or have questions about our SMS communications, may reply “HELP” to any SMS message they receive from us, or contact our customer support team at care@kavahealth.com.
  • Compliance with our Privacy Policy. All phone numbers will be handled in accordance with this Privacy Policy. Further, note that Kava Health does not share SMS opt-in consent with any third parties for solicitation purposes.

We retain your information only as long as necessary to fulfill the purposes outlined in this policy or comply with legal obligations. Once no longer needed, data is securely deleted or anonymized.
